Disability Insurance
We offer Disability Insurance through New York Life that replaces a portion of your income in the event of illness or injury. Teammates are eligible for Disability Insurance on the first of the month following 180 days of employment.
Company Paid Short-Term Disability
Short-Term Disability coverage pays you a benefit if you temporarily can’t work because of an injury, illness, or maternity leave. Benefits may be reduced by other sources of income, such as state disability, sick pay, workers’ compensation, Social Security, etc. Upon meeting eligibility requirements, you will be automatically enrolled in this coverage at no cost to you.

**Maximum payment period is based on the first day benefits become payable, not on the first day you are disabled.
Long-Term Disability
Following 180 days of employment, you will have 31 days to elect this benefit, which you pay for.

Benefits may be reduced if you have other sources of income, such as state disability, sick pay, etc.
*Depending on your age at the time of disability, your benefits may be subject to a different schedule. Refer to the Maximum Benefit Period table, which can be found in your plan documents.
Evidence of Insurability
Long-term disability may require evidence of insurability (EOI). This means you’ll be asked to answer medical-related questions. After electing coverage, you will receive more information if EOI is required.
