Eligibility
As an Owens & Minor teammate, you’re eligible for benefits if you work 30 hours or more per week.
Hawaii teammates who work 20+ hours per week and earn a monthly wage of at least 86.67 times the Hawaii minimum hourly wage are eligible for medical coverage that starts after 30 days of employment.
Your Eligible Dependents
During enrollment, you’ll be asked to verify that your dependents meet the eligibility requirements for coverage. Eligible dependents include:
Your Spouse.*
The individual you’re legally married to.
Your domestic partner.*
Must be 18+ years old; unmarried; not blood-related; mutually supporting; share a common residence; and be each other’s sole domestic partner.
Your qualified children under the age of 26.
Biological, step, legally adopted, your domestic partner’s children, or lawfully placed with you for legal adoption and/or eligible foster children.
Your unmarried child who is disabled: Living with you; dependent due to a mental or physical disability.
*Spouses or domestic partners who have access to other employer insurance cannot enroll under O&M medical or prescription plans.
Dependent Eligibility Verification
Domestic partners
For your domestic partner and their eligible children to receive benefits under the Owens & Minor Welfare Benefits Plan, both you and your partner must:
- Complete and sign the Declaration of Domestic Partnership (the “Declaration”).
- Upload the signed Declaration and appropriate documentation to Workday HR within 31 days of hire/Qualified Life Event or by the end of the Open Enrollment period.

All other dependents
Owens & Minor reserves the right to audit dependent eligibility. Be sure to keep proof of your dependents’ eligibility in case of an audit. Refer to the Documents Library for a list of supporting documents.
